Here is the checklist of the documents needing to be filed under the Loss of License policy.

HRSS and Pilot

HRSS to provide the form to pilot|| Both the pages to be filled and signed by pilot and submit the scan copy to HRSS

Intimation mail sent to DGCA by Company Doctor (Applicable only for scenario 2, refer slide 6)

HRSS

HRSS can coordinate with company doctor in order to get the mail

Copy of License & Civil Aviation Dept Certificate

Pilot

Scanned copy of all the pages to be submitted to HR by pilot

Employer Certificate/ Salary certificate

HRSS

Last 6 months salary slip to be provided by HRSS

Doctor's certificate /Prescription

Pilot

Treatment related documents|| Certificate from private doctor or supporting documents to support the end to end TMU period (Scanned PDF copies needs to be send to HRSS)

Civil Aviation Medical Certificate

Pilot

Scanned copy of all the pages to be submitted to HRSS

TMU or PMU certified by DGCA

Pilot

Scanned copy of all the pages to be submitted to HRSS

No flying certificate from company

HRSS

HRSS will prepare and send to insurer along with above documents
